Personalised stratified follow-up (PFSU) is an approach to follow-up care that is tailored to individual needs, rather than relying on routine outpatient appointments for everyone.
PFSU supports people to feel confident and supported after cancer treatment, while ensuring timely access to clinical care when it is needed.
How PFSU works
PFSU may include:
- Supported self-management
- Remote monitoring of symptoms or test results
- Rapid access to clinical advice if concerns arise
Follow-up is based on risk, need, and personal preference, rather than a fixed schedule.
