To support improving equitable access to patients across our footprint, Thames Valley Cancer Alliance have developed updated Pan Alliance Urgent Suspected Cancer (USC) referral proformas to support GP referrals to secondary care:
- Urgent Suspected Cancer Form: Breast
- Urgent Suspected Cancer Form: Gynaecology
- Urgent Suspected Cancer Form: Lung
- Urgent Suspected Cancer Form: Lower Gastrointestinal
- Urgent Suspected Cancer Form: Prostate
- Urgent Suspected Cancer Form: Skin
- Urgent Suspected Cancer Form: Upper Gastrointestinal
Work to develop forms for other pathways is underway.
