This document describes the redesigned stratified follow-up pathway for colorectal cancer patients. It defines the three strata of follow-up support available to this cohort of patients – supported self-management, face-to-face follow-up and end of life care. It outlines which individuals are eligible for entry onto the supported self-management pathway (SSMP) for their aftercare.
