Prostate Cancer stratified follow up pathway and guidelines

These documents (adapted from London Cancer) describe a stratified follow-up pathway for men with prostate cancer. Patients with stable prostate cancer after treatment without significant problems are offered the chance to enter supported self-management (SSM) and receive results of surveillance tests by telephone or letter but not in a face to face consultations. Providing all remains well, patients are discharged from SSM after three years into primary care.

The intensity of stratified follow-up is proportionate to risk of recurrence and late effects
(physical/psychosocial). Conceptually, there are three categories based on risk and follow up is proportionate to that risk.
